Exercise Referral

Discover the empowering benefits of physical activity through our Exercise Referral program.

Exercise Referral is a scheme to introduce those living with a long-term health condition to the benefits of regular physical activity.

Following a referral from your doctor or health professional, we will assess your eligibility to join the scheme.

If eligible you will be offered an initial assessment with one of our Exercise Referral team to discuss your needs.

During this assessment they will prescribe a suitable exercise program, or activities to participate in, that can help you to manage your health condition. To ensure progress, we'll stay in touch with you at regular intervals—specifically at 6 and 12 weeks—to monitor your achievements.

Who can attend?

To be eligible to join the scheme you need to be referred by your doctor or health professional and have a long-term health condition or injury.

Please bear in mind appointments are limited and you may have to wait longer for evening or weekend time slots.  Appointments can take up to two hours.

If you already have a BH Live Active membership you may not be eligible for this scheme. You may be offered an alternative pathway for support.
