Guidance for Healthcare Professionals

Refer patients onto our Exercise Referral Scheme who have a long-term health condition, or are recovering from illness or injury.

Includes:

  • An initial consultation lasting up to two hours with an Exercise Referral Advisor
  • Personalised exercise programme
  • A gym induction
  • Access to gym, swimming pool, and classes at BH Live Active centres
  • Reviews midway and on completion of the scheme.

Customers also benefit from twelve-weeks of reduced pay-as-you-go fees for activities. Alternatively, customers can take up a GP referral membership for a minimum of 3 months, rolling on for as long as needed.

N.B. People in receipt of benefits can qualify for reduced prices for a twelve-month period.

Customers can access any of our activities, across any of our sites, including our specialist exercise referral classes such as hydro, health circuits, nice & easy, chair based, and those for wellbeing.

BH Live Active centres offering referrals across the region include Littledown, Pelhams Park, Stokewood and Corfe Mullen in Dorset, and Mountbatten and Pyramids in Hampshire.

A one-to-one consultation includes:

  • An explanation of the scheme
  • A thorough review of the patient’s medical history
  • A review of motivational issues and time for questions
  • Identifying the patient’s personal goals
  • Relevant assessment measures (e.g., BP & HR, weight, BMI, height, waist circumference, and range of movement tests and/or an exercise tolerance test if relevant)
  • Exercise prescription based on medical status, personal needs, and goals
  • Gym induction if appropriate

Areas of our team’s expertise also include cancer rehabilitation, pulmonary rehabilitation, back care management, neurological conditions, obesity, and diabetes.

Contraindications to referral:

  • Unstable Angina
  • Poorly controlled heart failure
  • New or uncontrolled arrhythmias
  • Symptomatic hypotension
  • Febrile illness (temporary)
  • Resting or uncontrolled Tachycardia (RHRs >100 bpm)
  • Resting SBP >180 and/or resting DBP >100
  • Unstable diabetes (poorly controlled/elevated blood glucose levels, repeated hypoglycaemic episodes)

Patients with high levels of frailty and/or significant functional limitations who need ongoing, hands-on one-to-one assistance at every visit must have a carer in attendance.
